Mary Elizabeth Cook Formy-Duval, 99, affectionately known as Beth, Granny, and Miss Beth, died peacefully surrounded by her family and friends on December 10, 2012. She was born on August 5, 1913, in Acme, N.C., daughter of the late John Daniel and Elizabeth Elmira Butler Cook. Beth was preceded in death by her husband, Charles Wilbur Formy-Duval, her sister, Eunice Herring, and her brothers Earl, Daniel, J.W., and Frederick, and the recent death of a special sister-in-law, Katie Cook. She is survived by her children Dorothy Thomas (Calvin), of Hyattsville, Md.; Judy Benson (Thomas) of Delco; Charles Formy-Duval (Gloria) of Acme, and Maurice Formy-Duval (Maryan) of Acme. She is also survived by ten grandchildren, eleven great grandchildren, and many nieces and nephews.

Beth was a homemaker who loved her Lord and Savior Jesus Christ, her family, friends and the simple things of life. A widow for 39 years, Mom relied upon and was thankful to God for meeting her daily needs. Mom loved reading The Bible, and was a whiz at crossword puzzles, cryptoquotes and other types of puzzles. Her gentle, compassionate and loving ways will be deeply missed.

The family will receive friends from 6:30 to 8:30 p.m. Wednesday, Dec. 12, 2012 at Wilmington Funeral Chapel

A service celebrating Beth’s life will be held on Thursday, Dec. 13, 2012, at 3:00 p.m. at Cape Fear Baptist Church in Acme. Reverend Dondi Pace and Beth’s niece, Reverend Patsy Bullard Smith, will officiate. Interment will follow in the church cemetery.

Pallbearers will be her grandsons: Charles Wilbur Formy-Duval II and Charles Thomas Grice; James and Richard Thomas; and Scott Williams and Michael Formy-Duval. Honorary pallbearers will be her nephews Carlos Bullard, Freddy Cook, Danny Herring, and W.T. Medlin and a special great grandson Cody Bullard.

The family gratefully acknowledges and thanks Faye, Mom’s weekday and longtime caregiver and Dolly and Lily, Mom’s weekend and weeknight caregivers, for their love, companionship, and wonderful care for Mom. A special thank you to the Lower Cape Fear and Community Hospice nurses and staff in Whiteville and Premier Living staff for your loving care for Mom.
